Have You Taken the Community Readiness Survey?

The Durham-Middlefield Local Wellness Council is conducting an anonymous survey on substance abuse required by a Drug Free Communities grant awarded to the organization in the amount of $500,000.

 

As someone who lives and/or works in the towns of Durham and Middlefield, you have undoubtedly developed a sense of community life here.  We are very interested in your perceptions of our town and the implications for the quality of life of our young people.

The Durham Middlefield Local Wellness Council is a coalition comprised of volunteers from the communities of Durham and Middlefield and staff from the Durham Middlefield Youth & Family Service, Inc., an agency that has been working to reduce high-risk behaviors such as underage drinking and other illicit youth substance use. The success of our efforts is, to a considerable degree, affected by the prevailing community attitudes regarding underage alcohol and other substance use.

We are inviting you to complete an anonymous survey regarding the "readiness" of the communities of Durham and Middlefield to support actions that enhance the well-being of our children.  Your name and/or e-mail address was randomly selected from publicly available lists such as the on-line Chamber of Commerce membership list, business websites and from DMYFS Community Email mailing list.

The results of our readiness survey will help us plan the future efforts of the Durham Middlefield Local Wellness Council by measuring current community perspectives regarding prevention. We hope you will take a few minutes to share your perceptions of the communities of Durham and Middlefield. Results from this survey will only be released in aggregate form in which no individual's response can be identified.
